填写队伍资料。国际天文联会鼓励队伍由包括系外行星科学家或专业天文学家(Exoplanetary Scientist(s)/Professional Astronomer(s))、业余天文学家(Amateur Astronomer)、天文爱好者(Astronomy Enthusiasts)、教师或教育工作者(Educators)以及学生或青年(Students and Young Adults)组成，但亦欢迎任何不少于2名，并属以上不同组别的人士组队参加。换而言之，队伍中包含最少一名老师及一名学生；或一名业余天文学家及一名天文爱好者即可。

在「Size of your team」中填上队伍人数，以及在「List all the affiliations/organizations that all the members in your group belong to」中提供各人所属的机构或组织（如有）。

选择举办的外展活动类型（可多选），包括：网上直播、社交媒体发文/ 活动、不设互动环节的现场活动（例如演讲等），以及设有互动环节的现场活动。如不属上述类型，请另作说明。

在「Number of public engaged in the outreach events」上填写所有外展活动总公众参与人数。

在「Describe your outreach initiatives in details (A maximum of 300 words)」上以不多于300字详尽介绍外展活动的内容特色，并在「Post photos/images of your outreach event(s)」中上载一些外展活动的照片或图像。如果照片或图像数量甚多，可先上载到云端硬碟，并在「Alternatively, you may post a URL to your photos/images of your outreach event(s)」中提供链结以供浏览。

选择命名提议所属的系外行星系统。国际天文联会强烈建议每支队伍为其命名提议选择一个系外行星系统，如选择通用提交，即最后一个选项，可能会在评选上造成不利。 （*必填）

填写主星 (Name of the host star in the exoworld system)及系外行星(Name of the planetary body in the exoworld system) 的命名提议。名字必须使用拉丁字母，而第一个字母须为大写，详情请参阅命名规则。 （*必填）

在「What is the language of the proposed name?」中提供命名提议所属的言语。 （*必填）

在「Describe the naming theme under which the name of the proposed exoplanet & its star fall under. The theme should be summarized in a sentence or two, and broad enough that additional names related to the theme could be used to name additional objects in that system in the future.」中，以一两句话来概括提议中系外行星及其主星的命名主题。主题需足够广泛，以便未来继续使用与该主题相关的名字来命名该系统中的其他天体。 （*必填）

在「Write a detailed description in English」中，以300字为限，用英语详细描述该命名提议的背后意义。 （*必填）

在「Upload a video of the presentation explaining the meaning behind the proposed name in its own language, with captioning in English. Maximum of 3 minutes.」中上载一段以3分钟为限的短片，透过母语解释命名提议的背后意义，并配上英文字幕。如果短片容量过大，可先上载到云端硬碟，并在「Alternatively, you may post a URL to your video presentation of your proposed name」中提供链结以供浏览。

按下「提交」即完成是次提名。
