填寫主要聯絡人的資料，包括英文姓名、電子郵件以及所代表的國家或地區。如隊伍希望代表香港，請在「所代表的國家或地區」中，選擇 「China, Hong Kong」。

填寫隊伍資料。國際天文聯會鼓勵隊伍由包括系外行星科學家或專業天文學家(Exoplanetary Scientist(s)/Professional Astronomer(s))、業餘天文學家(Amateur Astronomer)、天文愛好者(Astronomy Enthusiasts)、教師或教育工作者(Educators)以及學生或青年(Students and Young Adults)組成，但亦歡迎任何不少於2名，並屬以上不同組別的人士組隊參加。換而言之，隊伍中包含最少一名老師及一名學生；或一名業餘天文學家及一名天文愛好者即可。

在「Size of your team」中填上隊伍人數，以及在「List all the affiliations/organizations that all the members in your group belong to」中提供各人所屬的機構或組織（如有）。

選擇舉辦的外展活動類型（可多選），包括：網上直播、社交媒體發文/ 活動、不設互動環節的現場活動（例如演講等），以及設有互動環節的現場活動。如不屬上述類型，請另作說明。

在「Number of public engaged in the outreach events」上填寫所有外展活動總公眾參與人數。

在「Describe your outreach initiatives in details (A maximum of 300 words)」上以不多於300字詳盡介紹外展活動的內容特色，並在「Post photos/images of your outreach event(s)」中上載一些外展活動的照片或圖像。如果照片或圖像數量甚多，可先上載到雲端硬碟，並在「Alternatively, you may post a URL to your photos/images of your outreach event(s)」中提供鏈結以供瀏覽。

選擇命名提議所屬的系外行星系統。國際天文聯會強烈建議每支隊伍為其命名提議選擇一個系外行星系統，如選擇通用提交，即最後一個選項，可能會在評選上造成不利。（*必填）

填寫主星 (Name of the host star in the exoworld system)及系外行星(Name of the planetary body in the exoworld system) 的命名提議。名字必須使用拉丁字母，而第一個字母須為大寫，詳情請參閱命名規則。（*必填）

在「What is the language of the proposed name?」中提供命名提議所屬的言語。（*必填）

在「Describe the naming theme under which the name of the proposed exoplanet & its star fall under. The theme should be summarized in a sentence or two, and broad enough that additional names related to the theme could be used to name additional objects in that system in the future.」中，以一兩句話來概括提議中系外行星及其主星的命名主題。主題需足夠廣泛，以便未來繼續使用與該主題相關的名字來命名該系統中的其他天體。（*必填）

在「Write a detailed description in English」中，以300字為限，用英語詳細描述該命名提議的背後意義。（*必填）

在「Upload a video of the presentation explaining the meaning behind the proposed name in its own language, with captioning in English. Maximum of 3 minutes.」中上載一段以3分鐘為限的短片，透過母語解釋命名提議的背後意義，並配上英文字幕。如果短片容量過大，可先上載到雲端硬碟，並在「Alternatively, you may post a URL to your video presentation of your proposed name」中提供鏈結以供瀏覽。

按下「提交」即完成是次提名。
